Parent Permission for Cactus

Dear parents,

If your daughter travels to Mesa, AZ for Cactus Classic please fill out this form and give to Bella from 12Under (Rio) team or Bella's mom Karine (phone# 440-715-5504)

by Monday 02/12.

Please make sure you share any medical/dietary restrictions, as well as anything your chaperones should know about your child.

The form must be filled out even if you travel to Cactus yourself as a spectator or chaperone.
